NORTH ARBOR DENTAL ("we") uses an online booking form, provided by Sikka, to take appointment requests on this page and on our own website. This policy explains what we collect through it, why, and the choices you have.
What we collect
- Your name, email address and phone number.
- The treatment, date and time you choose.
- Your answers to the questions on the form, including insurance details if you give them.
- Basic technical information your browser sends with every request, such as your IP address, used to run and secure the service.
How we use it
- To review and confirm your appointment request and contact you about it.
- To add the appointment to our records.
- To send you messages about this appointment, such as confirmations and updates about your request.
Who we share it with
- Sikka, which provides the booking form and handles your information on our behalf. It does not use it for its own purposes.
- Advertising platforms, such as Google or Facebook, only if you arrived from one of their ads and accept advertising cookies: we let the platform know that its ad led to a booking or a call, using the reference it added to the ad link. Your name, email address and phone number are not shared.
We do not sell your information.

If you accept advertising cookies and later reject them, we delete them from your browser.
The booking form embedded on our own website does not set cookies or store anything on your device.
How long we keep it
We keep your information only as long as we need it to handle your request and to meet our obligations for patient records.
Your rights
You can ask to see the information we hold about you, ask us to correct it, or withdraw your consent to optional uses such as advertising measurement. If you are not satisfied with how we handle a privacy concern, you can contact the privacy regulator where you live.
